In May the U.S. Chemical Safety Board (CSB) released a “Factual Investigative Update” on the 3rd April 2017 catastrophic rupture of a pressure vessel at the Loy Lange Box Company that killed four people and left another in critical condition. Once again the CSB has demonstrated that it is possible to quickly publish and share the learnings from a major process safety incident investigation.
The image shows the base of the steam condensate pressure vessel and connected bottom pipe work. This base failed and blew out of the vessel.
This incident investigation report exemplifies what happens when pressure vessel fails and the stored energy is released. It should serve as reminder of the need to ensure that high quality asset integrity systems are being rigorously implemented.
